Your time at the Center for Vulvovaginal Disorders is very valuable. To make the most of your visit, it is absolutely essential to print out and complete some forms and questionnaires before your visit.
If you participate with Medicare (as either your primary or secondary insurance) you must print and sign our Medicare Waiver which confirms your understanding that we have opted out of Medicaire. If you participate with Tricare, you must print and sign our Tricare waiver which confirms that we do not participate with Tricare.
If you are a patient who will be coming to the Center for Vulvovaginal Disorders you must print out and complete the Vulvar Pain Questionnaire . Please allow 45 minutes to complete this very extensive questionnaire.
If you are a new patient who will only coming for routine gynecology appointment (Well Woman Exam) then you must fill out a General Medical History.
Directions
As your time with us is very valuable, we do not want you to waste it by getting lost. Therefore before you leave you house, please print out the following directions and Google Maps to our offices. Please allow plenty of time as traffic and parking can be difficult in Washington and New York. We emphatically recommend that you take public transportation to our offices in Washington and New York as we are very near the Foggy Bottom Metro stop in Washington and Grand Central Station in New York City.
